Welcome to The 青楼传媒 Patient Portal

The 青楼传媒 is an option for all of the clients served at the Counseling Center, Health Center and Ludden Speech and Language Clinic, that allows our patients to communicate securely with providers and office staff. Our Patient Portal is a great option for ALL of our patients, including 青楼传媒 students, 青楼传媒 staff/faculty, and community members. Our portal is completely HIPAA compliant.

When you have an account you have access to numerous features, including:

Messaging securely with providers and office staff:
- Receive lab results
- Ask providers questions
- Request past medical records, including immunization records
- Request a medication refill
- Billing questions
Verify upcoming appointments
Print immunization records
Upload documents, including immunization records and release of information requests
Enter insurance information
Reference educational materials given to you during visits

Creating an Account

If you had lab tests completed at the 青楼传媒 Health Center a portal account may have already been set up for you at your appointment. However, if you do not have an account please follow the instructions below:

Create an account using the guidelines below:

Community Members

Create your own username
Enter your email (this should match the email address that you put on your registration form during your visit)
Enter the 7 digit ID number that was given to you at your appointment (if you are unsure, please call the Health Center for assistance at 773-298-3712)
Enter your name exactly as it was entered into our system when you made your appointment (e.g. Kathleen Smith, not Kathie Smith)
After you submit your request, you will receive an email with a link you need to follow to create your password and access your account

Instructions for Immunization Upload

All 青楼传媒 students taking more than 6 credits per semester (at any point in their time at 青楼传媒) must comply with the state immunization requirements or they will have their registration blocked for subsequent semesters. To see a full list and descriptions of what is required, please see our Student Immunization Requirements page.

Please upload your full immunization record by clicking on the 'Upload' tab at the top of the screen.
You will receive a secure message back from a provider stating that you are either compliant or that further records are needed. If you do not receive a message back in one week, please call to confirm that we received your records.
